Being healthy as possible in adult life
This pathway looks at the things young people might need to consider when moving from children’s to adult health services, including:
- how they can take care of their health and wellbeing
- mental health
There are many changes to consider, especially regarding how health and care are managed. This involves:
- reviewing services that may not be available once a young person turns 18
- arranging alternative healthcare options
To help people with additional needs stay healthy, everyone aged 14 or over on their doctor's learning disability register is entitled to a free annual health check to help find health problems early and keep them well.
