Annual EHCP review

How to complete annual review forms.

Education, health and care plans (EHCPs) are reviewed every year to record progress towards outcomes.

These ‘annual reviews’ must take place within 12 months of an ECHP (also known as an EHC plan) being finalised or since its last review.

The reviews are part of the statutory process looking at the needs, provisions and outcomes specified in an EHCP. They are also a way to raise concerns or suggest changes to an EHCP.

Review form guidance

As part of the statutory review process, you must send us an annual EHCP review form no later than 2 weeks after an annual review has taken place. It can be used to:

  • record suggested changes to an EHCP
  • ask for a change of placement
  • ask for additional levels of support

In some circumstances, they can also be submitted as evidence towards a SEND tribunal.

Pilot and review

Our annual review guidance and forms are currently being reviewed as part of a pilot.

They will be replaced by the end of the academic year 2023 to 2024.
